Fonction
En tant quassistant social, votre mission principale sera de rencontrer, écouter et recueillir toutes les informations utiles concernant les patients hospitalisés.
Missions spécifiques :
- Conseiller, orienter et soutenir les patients et leurs familles
- Aider les patients dans leurs démarches pour optimiser le retour à domicile, en recherchant leur plus grande autonomie
- Travailler en réseau, en partenariat et en équipe interprofessionnelle
- Produire un rapport dactivité et rédiger des notes administratives et courrier
- Services : pneumologie, revalidation, hôpital de jour gériatrique
Profil - Disposer dune expérience en milieu hospitalier
- Connaître la région et le réseau
- Pouvoir travailler de façon autonome et prendre des initiatives
- Disposer de qualités relationnelles et avoir le sens de la communication
- Etre disponible et flexible
- Avoir lesprit critique
- Etre motivé et dynamique
- Etre disponible rapidement
